Occupational Therapy (OT):
Revive Occupational Therapy (OT) focuses on helping adult clients with neurological deficits re-gain participation in the activities that are most meaningful to their day-to-day lives. Services are delivered in the environment that best fits our client’s needs: in their home and/or in their community.
Individual evaluations and treatment plans can address the following:
- Upper extremity motor control and sensation
- Fine motor skills
- Vision
- Functional cognition
- Self-care skills
- Community reintegration
- Adapted equipment needs
- Home modification recommendations
Payment:
Medicare: Revive OT is an approved Medicare provider. Therapy visits are reimbursed through Medicare Part B for certain qualified services.
Private Insurance: Services may be covered via out-of-network, per your insurance plan
Private-pay: Cash-based options are available.
Doctor referral:
A doctor referral is needed if paying for services through insurance. Private-pay clients do not need a doctor referral.
